Hospice Team Physician

We are looking for a dedicated physician to join our hospice care team and provide comprehensive medical support to individuals facing serious, advanced, and life-limiting conditions. The Hospice Team Physician plays an important role in helping patients remain comfortable while supporting their personal goals, dignity, and quality of life.Working as part of an interdisciplinary care team, this physician partners with patients and families as well as nurses, advanced practice providers, attending physicians, and other clinical professionals. The physician provides medical direction for symptom management, medication decisions, care coordination, and ongoing hospice planning.This role also offers opportunities to consult with community-based providers, contribute to clinical education, and help strengthen the delivery of high-quality hospice services.Role DetailsFull-time physician opportunity located in Dayton, OhioResponsible for hospice coverage across the Southern Ohio regionServices may be delivered in patients' residences, nursing or other care facilities, and hospice inpatient locationsClinical responsibilities include both face-to-face visits and telehealthParticipation in weekend and on-call schedules is requiredPhysician must satisfy all applicable credentialing requirements within the Southern Ohio service areaPeriodic travel to other locations within Ohio may be necessaryWhat You’ll DoProvide medical evaluation and ongoing management for hospice patients with complex and life-limiting conditionsEstablish, modify, and oversee patient-specific treatment and hospice care plansAssess and address pain, symptom burden, and other clinical concernsEvaluate medication regimens and make adjustments when medically indicatedContribute to interdisciplinary team conferences and coordinated care planningCommunicate and collaborate with attending providers, nursing professionals, APPs, and other members of the care teamOffer medical supervision, consultation, and clinical support to advanced practice providersPromote complete, accurate, and timely clinical documentationServe as a resource for hospice clinicians and participate in staff educationEnsure medical records appropriately document terminal diagnoses and continued hospice eligibilityParticipate in clinical quality initiatives, process improvement, and other organizational programsWhat You’ll BringDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degreeUnrestricted medical license in the State of OhioCurrent, unrestricted DEA registrationPrevious experience in hospice, palliative care, hospital medicine, or a comparable clinical setting is preferredBackground caring for patients with advanced, complex, or terminal conditions is preferredBoard certification or eligibility in Hospice and Palliative Medicine is preferredCurrent CPR certificationExcellent clinical decision-making and interpersonal communication abilitiesProficiency with electronic health records and standard computer applicationsValid driver's license and acceptable driving recordAutomobile insurance that satisfies applicable requirementsAbility to travel among patient care locations when necessaryAvailability for assigned weekend and on-call responsibilitiesAbility to successfully complete required employment screening and background verificationCompensation and BenefitsWe believe physicians and clinical professionals provide the best care when they have strong support behind them.
