Mental Health Specialist: ACT Team & Crisis Support

Horizon House, Inc. in Claymont, Delaware seeks an Associate Mental Health Specialist to join the ACT multidisciplinary team. This role provides rehabilitation, support services, crisis intervention, and coordination with families and community agencies to ensure integrated treatment plans. Responsibilities include developing treatment goals with participants, coordinating with employment services, and offering on-call crisis intervention under supervision.
