Caregiver Support Program Clinical Assessor, Registered Nurse, Critical Care

The Caregiver Support Program Clinical Assessor, Registered Nurse, Critical Care duties include, but are not limited to the following:

• Assesses, plans, implements and evaluates care based on age-specific components.
• Delivers fundamental knowledge-based care and counseling to assigned clients while developing technical skills.
• Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
• Follows procedures per established policies and guidelines.
• Influences care outcomes by collaborating with members of the interdisciplinary team.
• Executes position responsibilities that demonstrate beginning leadership skills and participates in specific PCAFC experiences.
• They demonstrate creative approaches to the management of aspects of complex patient care.
• Provides services of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, and health counseling.
• Possesses thorough knowledge of PCAFC, PGCSS, clinical documentation requirements, current standards of care, and all regulatory compliance requirements and guidelines.
• Accountable for utilizing critical thinking skills and sound judgment to ensure Veterans and caregivers are provided with thorough, comprehensive care options within the scope of the CSP.
