We're seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our Home Health team on a PRN basis. As a valued member of our care team, you'll provide hands-on support to patients in their homes, helping them maintain independence, dignity, and quality of life.

What You'll Do

In this role, you'll work closely with nurses and other healthcare professionals to deliver exceptional patient care in a home setting. You'll assist patients with daily activities, support their physical well-being, monitor changes in condition, and help create a safe and comfortable environment.

Your impact will include:

• Providing personal care and assistance with daily living activities
• Supporting mobility, transfers, and safe ambulation
• Monitoring patient well-being and communicating concerns to the care team
• Recording care activities and patient information accurately
• Assisting with vital signs, meal preparation, and light household tasks
• Building positive relationships with patients and families
• Following safety, infection control, and confidentiality standards
• Traveling throughout the assigned service area to provide care

What We're Looking For

✅ Active Texas CNA Certification

✅ High School Diploma or GED

✅ At least 1 year of experience in healthcare, home health, long-term care, or a related setting

✅ Valid Texas driver's license, reliable vehicle, and current auto insurance

✅ Strong communication and interpersonal skills

✅ Ability to work independently while following established care plans

✅ Current CPR certification (or willingness to obtain upon hire)

✅ Commitment to providing compassionate, patient-centered care

Preferred Experience

⭐ Home Health or Hospice experience

⭐ Experience caring for seniors, disabled individuals, or chronically ill patients

⭐ Bilingual English/Spanish skills

⭐ Familiarity with EMR systems

⭐ Additional caregiving or patient care certifications

⭐ Knowledge of home healthcare best practices and regulations
