Hospice RN Case Manager: Compassionate Care Leader

Hospice of New Jersey is seeking a full-time Hospice RN Case Manager (RNCM) to join our nursing team. You will provide hands-on hospice nursing care and coordinate services across the community with a compassionate approach. As part of our interdisciplinary team, you will educate patients and families, guide care plans, and ensure comfort and dignity at the end of life. Requires RN license and 1 year med-surg/acute care experience. #J-18808-Ljbffr Hospice of New Jersey
