Job Title Nurse Practitioner – Adolescent Health (School Based Health Center) – Mount Sinai Hospital – Full Time – Day Job Summary A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. NPs are independent practitioners who diagnose, treat, and prescribe for a patient’s condition that falls within their specialty area of practice. Principal Duties and Responsibilities Assess the health status of patients/families through comprehensive health history and physical examination. Initiate and order selected therapeutic and diagnostic procedures, consultations, laboratory examinations, and prescribe medication regimens, interpreting findings and adjusting the plan of care according to accepted standards and guidelines. Educate and counsel on health promotion, disease prevention, and management of acute and chronic illnesses. Document findings and plan of care in the patient’s medical record. Consult with physicians according to established standards, policies, procedures, and protocols. Demonstrate knowledge and skills to provide age‑appropriate care, considering physical, psychosocial, educational, and safety criteria. Initiate referrals to specialty services and foster continuity of care. Participate in team meetings and conferences to enhance interdisciplinary care delivery. Maintain patient confidentiality in the management of information. Develop and monitor individualized care plans, recording, reporting, and interpreting client responses. Perform patient record reviews with the collaborating physician, not less than every three months if using a collaborative practice agreement. Complete required professional practice evaluations (FPPE or OPPE) as required for continued credentialing. Practice as a member of the allied health staff in accordance with the Medical Staff rules and Hospital Staff bylaws. Professional Development and Practice Standards Support the mission, vision, philosophy, and goals of the Department of Nursing. Demonstrate accountability for own nursing practice and adherence to ethical principles and the code of ethics. Show a humanistic, kind, and caring attitude in delivering health care. Serve as a resource for nurses and other health team members. Participate in peer review, research studies, and integrate research findings into practice. Maintain current knowledge through self‑directed learning, literature review, and educational programs. Contribute to maintaining a positive image of advanced practice nursing and to positive outcomes. Treat co‑workers, patients, and families with dignity and respect. Knowledge, Skills, and Abilities History taking and physical assessment techniques. Documentation of competency in specialty procedures per standards during orientation and annual reviews. Interpretation and application of nursing research. Case‑load management and coordination. Collaboration within a Collaborative Practice Agreement or Relationship Attestation. Physical stamina and manual dexterity. Excellent interpersonal, communication, critical thinking, and leadership skills. Education and Credentials New York State RN license with current registration. State certification as a Nurse Practitioner (New York State Department of Education or national association). Graduate of a New York State registered Nurse Practitioner program or equivalent or certified via national exam. Completed required pharmacy component and coursework in prescription laws. Collaborative Practice Agreement with a physician if less than 3,600 practice hours; existing NP clinical practice qualifies otherwise. Master’s degree or post‑master’s NP certificate approved by the institution. Experience and Additional Requirements Previous progressive experience relevant to adolescent health practice. National Board Certification in Advanced Practice Nursing required (may be deferred up to one year with approval). Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification. Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), Neonatal Resuscitation Program (NRP) certificates as required by specialty.
