RN Care Manager - Temporary Hire

City/State: Nyack, New York Department: Care Management Work Shift: Day (Mon-Fri, rotating schedule) Scheduled Hours: 8:30 AM-5 PM (8 h daily, 40 h per pay period) Pay: Starting at $125,000 Job Summary Collaboratively implements and facilitates patient care plans from pre-hospital to post-discharge while managing an assigned unit case load. Responsible for assessment, planning, and monitoring options and services to provide an individualized plan for the patient across the continuum of care. Uses independent judgment and discretion, and continually reassesses the continuum as an ongoing process for the patient. Applies clinical knowledge, communication skills, problem-solving, conflict-resolution, and strong assessment, organization, and time-management skills. Enforces hospital policies and procedures for appropriate and timely clinical care, and recommends disciplinary action when care is below standard. Essential Functions Learn all aspects of the position, including revised methods, within appropriate time frames; attend in-services and educational programs; identify self-development needs and use educational resources. Identify and support delivery of positive customer service to all patients, visitors, families, physicians, staff, etc.; conduct all interactions confidentially, courteously, professionally, and timely; recognize and respond appropriately to the needs of all customers. Conduct accurate assessments of patients; determine appropriateness of admission, continued stay, potential for discharge planning interventions and level of care based on approval criteria and standards; communicate with physicians regarding documentation; review the patient’s plan of care in conjunction with the clinical pathway and individualize outcomes. Monitor the appropriateness and quality of patient care; refer identified quality issues to the Performance Improvement Department; participate in concurrent quality audits; maintain knowledge of payer requirements and regulations; act as patient advocate, negotiate services with third-party payers, resolve patient complaints, and make referrals within one working day. Participate in interdisciplinary rounds; develop physician-directed plan of care for post-hospital services; initiate home-care referrals and arrange supplies; plan transfers to nursing facilities, rehab, etc.; involve patient/family in decision-making regarding post-hospital care and inform them of insurance coverage, co-payments, and Medicare/Medicaid guidelines. May take orders from physicians including discharge orders. Qualifications Bachelor’s Degree preferred or Associate Degree required. 4-6 years nursing experience in inpatient med-surge, ICU, or ER units preferred. Membership in a professional organization preferred. Effective written and verbal communication, decision-making, problem-solving, and comprehensive assessment skills (medium proficiency).
