Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient’s needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that allow for effective return to community activities. This role also serves as an educator of patients, families, students, coworkers, and self while adhering to the organization's Mission Statement and Core Values.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on progression towards discharge.
• Document care in a clear, concise, accurate, and timely manner as per clinic policies.
• Provide care under signed, valid physician orders and within insurance approval.
• Participate in the student program as assigned, fostering a positive learning environment.
• Actively create and pursue a professional development plan with clinical management.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand and engage with the organization’s performance improvement methodology.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• CPR Certification.
• Active and applicable state license for physical therapy.
