Role Description Concerto is seeking a PRN Licensed Practical Nurse to join its team. The PRN Licensed Practical Nurse functions as part of the hemodialysis health‑care team in providing safe and effective dialysis therapy for patients. The role works under the supervision of a registered nurse in accordance with Concerto’s policies, procedures, and training and in compliance with regulations set by state and federal agencies. Responsibilities Direct Patient Care – Provides hands‑on care to assigned patients under the supervision of a licensed nurse, including initiating and discontinuing dialysis treatments per prescribed orders. Vital Signs & Monitoring – Obtains pre- and post‑treatment vitals, weight, and vascular access assessment. Reports any unusual changes or findings to the registered nurse. Dialysis Machine Operation – Sets up, operates, and performs safety checks on hemodialysis machines; administers heparin as permitted by state law. Initiates and discontinues dialysis treatment according to prescribed orders and policy. Documentation & Lab Work – Accurately documents patient treatment details including vital signs, treatment parameters, and changes in condition, and prepares laboratory specimens as prescribed. Customer Service & Environment – Delivers excellent service to patients, families, and staff, while maintaining a clean and safe work environment. Qualifications Compassionate and empathetic Critical thinker and problem‑solver Individual who thrives in a team environment Individual with excellent communication skills Purpose‑driven Required Licensed Practical Nurse required Current CPR certification or completion upon starting in unit Able to meet physical demands of the position Minimum of (1) year of Licensed Practical Nurse experience in dialysis Physical Requirements & Work Environment This role requires successful completion of the Ishihara Color Blindness Test to ensure accurate color perception in a clinical setting. The position involves prolonged walking, standing, stooping, and bending, with the ability to lift up to 25 lbs. Strong focus and attention to detail are essential. The work environment includes temperature control, moderate noise levels, and potential exposure to infectious materials, requiring mental, visual, and auditory acuity.
