Brookhaven Hospice is seeking a self-motivated and compassionate Registered Nurse Admissions (Full-Time 30 hours/week) to join our growing team. This hospital-based position plays a key role in supporting patients and families during the most critical moments of care. As our Admissions Nurse, you'll be the face of our organization in hospitals and inpatient facilities, educating, assessing, and supporting patients and caregivers while collaborating with hospital teams to provide timely and appropriate hospice services.

Location: Daily Rounding in Southern NH Acute Care Hospitals

Schedule: Day shift, some late afternoons possible as ER/ICU rounding is part of this role. 5 6-hour shifts (1 must be Saturday). Flexible start and stop times possible for the right candidate provided it coordinates with hospital and hospice coverage needs.

Required: Active RN license & hospital bedside experience

Preferred: ICU, ER, Hospice or Palliative

Responsibilities

• Determines primary decision makers and pro-actively builds positive, solid working relationships with members of the medical community and partners.
• Daily Rounding in hospital ER, ICU and inpatient units on end of life patients and families for goals of care education.
• Admission assessment and transitioning patients to hospice.
• Promotes and positions Hospice's patients care service through regular visits in a manner that illustrates and reflects the Hospice's quality of care and unique capabilities.
• Works to build the brand of the hospice as the preferred hospice provider in our service area to achieve and exceed agency goals for growth and census management.
• Makes regular, planned calls to secure new referrals, build census, and meet established goals on new and existing, referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers.
• May help Hospice in determining patient eligibility, based on their hospice knowledge and training.
• Conducts initial meeting with the patient and family giving an overview of hospice service and philosophy and obtains consents for care.

Qualifications

• Bachelor's degree in nursing or health care preferred.
• Holds current unencumbered license as a registered nurse.
• Minimum one (1) years of hospital nursing experience required.
• Minimum one (1) years of sales/marketing experience in healthcare preferred.
• One (1) to two (2) years' experience in hospice/palliative strongly preferred.
• Skilled at establishing/maintaining working relationships with key staff, marketing contracts, and clinical staff to ensure thorough understanding of Hospice services.
• Proven ability to generate and maintain relationships with new and existing accounts including physicians, healthcare professionals, and community agencies.

$42.00 - $47.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate's relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
