Responsibilities:

• Provide high-quality, patient-centered care to residents in our post-acute rehabilitation and long-term care settings
• Administer medications, treatments, and procedures under the supervision of a registered nurse
• Monitor and report changes in resident condition to the care team
• Collaborate with healthcare professionals to develop and implement individualized care plans
• Maintain accurate and detailed records of resident care and treatments
• Provide emotional support and assistance with activities of daily living (ADLs) as needed
