Hospice Registered Nurse

About This Role The Hospice Nurse Weekend (RN) provides skilled nursing care to hospice clients in their homes or community health settings, ensuring high-quality, cost-effective care. This role is ideal for compassionate nurses who want to make a real difference in the lives of patients and their families. Highlights Location: East Hartford, CT Department/Unit: 95005-Hartford Hospice Gen Schedule: Part Time, Day shift What You'll Do Provide skilled nursing care to hospice clients in home or community settings. Review referrals and initiate contact with clients and families. Conduct complete nursing assessments based on referral data and clinical observations. Develop care plans in collaboration with clients and interdisciplinary teams. Facilitate peaceful death and ensure appropriate bereavement follow-up. Modify care plans as conditions change. Utilize interdisciplinary team members to support patient/family needs. Teach patients and reinforce caregiver education. Direct LPNs and Home Care Aides in executing the care plan. Participate in client care conferences and staff meetings.
