Physical Therapist - Home Health

Magenta Care is a Washington-licensed home health agency, nurse-founded and caring for patients at home since 2022. We are launching home health physical and occupational therapy in King County and are looking for experienced home health physical therapists who can hit the ground running.

Per-visit pay (W-2, per diem):

• Evaluation / start of care: $130
• Routine visit: $90
• Re-evaluation / progress visit: $105
• Discharge visit: $95

Responsibilities

• Provide, coordinate, and oversee skilled intermittent physical therapy in the home setting.
• Perform initial and ongoing assessments to determine patient needs and rehabilitation goals.
• Educate patients, families, and caregivers on therapeutic exercises, safety precautions, and functional maintenance strategies.
• Update and revise the plan of care (POC) in response to changes in patient condition, physician orders, or treatment goals.
• Document patient visits, progress, and outcomes in a timely and compliant manner using our electronic health record system.
• Communicate proactively with the patient's physician and case manager regarding clinical findings and recommendations.
• Conduct follow-up evaluations within 48 hours after significant health changes or hospital discharges (of 24+ hours), or as ordered by the physician.
• Maintain and protect patient confidentiality in accordance with HIPAA and company standards.

Requirements

• Active Physical Therapist license in the State of Washington (required).
• Minimum of 3 years of recent home health physical therapy experience, including independent evaluations and plans of care (required).
• Doctorate or Master's degree in Physical Therapy (Bachelor's degree acceptable with strong experience).
• BLS Certification (Basic Life Support) from an approved provider.
• Valid Washington State Driver's License and proof of auto insurance.
• Reliable transportation for in-home visits.
• Strong interpersonal skills and a patient-first mindset.
• Comfortable using mobile devices and electronic documentation systems.
