Palliative Care Physician Pennsylvania - PittsburghOur client is seeking a full-time Lead Palliative Care Physician to join their team. This physician will be a specialist in the community and care for patients with chronic kidney disease, end stage renal disease, and other polychronic conditions. The Lead Palliative Care Physician will work collaboratively with a team of providers and a market Medical Director, and primarily visit patients in their homes and via telehealth.RequirementsDoctorate Degree in MedicineBoard Certified in Hospice and Palliative MedicineComfort and skill in advance care planning and in goals of care discussionsExperience with Medicare Advantage and managed Medicaid populations, and commercial populationsMust have privileges at a local hospital, if you have connections with a local hospice, that is a plusExpertise in managing medication panels for complex patientsCurrent state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare.Willingness to become licensed in multiple states as company and patient needs requireMust have reliable transportation
