Provide physical therapy interventions to improve strength, mobility, and functional abilities. Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications. Collaborate with the interdisciplinary team, including occupational therapists, nurses, and physicians, to coordinate care and optimize patient outcomes. Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes. Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies. Required Qualifications Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program. Valid Physical Therapy license in the state of California. Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment. Excellent communication, interpersonal, and problem-solving skills. Ability to work both independently and collaboratively in a team environment. Preferred Experience / Skills Experience working in a skilled nursing facility or similar healthcare setting.
