The LCC will be working as part of our multidisciplinary team with an emphasis on our chronic patients, with conditions such as diabetes, cardiovascular disease, substance abuse and depression. May include traveling to our various sites.Essential Job Functions and Requirements:Our therapists provide individual and group counseling services for adults, children and family therapy. This includes but is not limited to:Participating in interdisciplinary treatment planningEvaluating data to identify causes of problem of individuals and to determine advisability of counseling or may refer to other specialist or institutionsCounseling or therapeutic interviewing to assist individual to gain insight into personal problems, define goals, and plan action reflecting interest, abilities and needFollowing up on results of counseling to determine reliability and validity of treatment usedDocumenting in medical record according to requirements of company and regulatory agenciesProviding crisis intervention consultationOther areas in which our therapists participate:Providing consultation/education to community service clubs, organizations, human services agencies, hospitals, and various county departmentsProgram development and staff inservicesPerforming administrative functions consistent with the needs of the programParticipating in staff meetings as requiredMeet monthly productivity requirement and assigned goalsProvide the best customer care possible, and the ability to be reliable working a flexible schedule as neededIdentify and build upon the strengths of consumers, coworkers, and the communities we serveSupport consumers' steps towards recovery and wellnessCreate an organizational culture that respects and celebrates the diversity of our consumersValue learning as an ongoing process that enables us to better serve our communitiesResearch and utilize our industry's best practices and analyze our own services to ensure the best possible outcomesQualificationsInvolvement in the Medication Assisted Treatment (MAT) ProgramOur Medication Assisted Treatment Program assists people with attaining and maintaining recovery from opioid dependence. When appropriate, we prescribe Suboxone to provide Opioid dependence treatment for patients with chronic pain disorders or for patients whom are dependent on Opioids and have been chronically treated for pain with Opioids. This involves a team approach with the staff at Aria Community Health Center supporting each patient's recovery or therapy for dependence or both through use of prescription medications, counseling, group sessions, case management services and other services to provide a whole patient approach. These treatments work together to assist patients in reducing the cravings and the other symptoms related to withdrawal.Minimum QualificationsMaster's in psychology, counseling or other related field from an accredited college or university.Must be licensed as a LCSW/LMFT, ASW/MFT, or LPCC in the state of California.Organized, self-driven, team player with technical skills.An understanding of the cultural sensitivities in this type of work and the use of cultural appropriate methods is important.Spanish speaking is preferred but not required.Experience working with farm worker communities and under-served populations is a plus.PHYSICAL/COGNITIVE REQUIREMENTSThe physical and cognitive demands listed below represent essential functions of this position and are required with or without reasonable accommodation:Hearing: Sufficient to engage with patients and staff in person and by telephone.Speaking: Clear verbal communication to convey medical and operational information.Vision: Visual acuity adequate for reading printed materials and viewing electronic screens.Cognitive Abilities: Aptitude to complete tasks, including abilities such as learning, remembering, focusing, categorizing, and integrating information for decision-making, problem-solving, and comprehending.Motor Function & Mobility: Ability to move within the work environment to complete tasks and interact with others.Occasional lifting and carrying of items up to 10 pounds without assistance.Frequent standing, sitting, bending, reaching, and other movements typical of a standard workday.Regular use of hands and fingers for typing, writing, operating tools or equipment, and handling materials.Some pushing and pulling required during daily tasks.BenefitsFour or five-day work weekVacationSick timeMedical/Dental/Vision planPaid CE (Continuing Education)403(B) retirementLife InsuranceNHSC (Loan Repayment Program)National Health Service CorpsACHC is a National Health Service Corps (NHSC) certified site that offers tax-free loan repayment assistance to support qualified health care providers who choose to take their skills where they are most needed.
