To empower members in managing their diabetes, the full-time remote Certified Diabetes Educator will guide a panel of patients through their healthcare journey, providing education and counseling via virtual communication to help them achieve self-management and clinical stability. Key responsibilities Screen members for eligibility in the Diabetes Clinic and conduct detailed assessments Provide ongoing education on diabetes management, including medications, glucose monitoring, and lifestyle counseling Collaborate with healthcare team members to develop care plans and optimize medication regimens Required qualifications Active unrestricted Compact Registered Nurse license or Registered Dietitian license Active Certified Diabetes Care and Education Specialist (CDCES) certification At least 2 years of experience in clinically managing diabetes
