Job Opportunity

Position Overview

We are seeking a dedicated healthcare professional to join our team as an Acute Care Case Manager specializing in Discharge Planning. The ideal candidate will have a minimum of 2 years of experience in acute care settings, with a proven track record in managing patient care transitions and delivering high-quality patient support.

Key Details

• Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)
• Start Date: ASAP
• Years of Experience Required: 2 years in Acute Care Case Management/Discharge Planning
• Weekend Requirement: Every other weekend
• Location: City, State

Qualifications

• Must possess a valid California state license (pending license accepted, but must be active upon start)
• Extensive hospital experience; behavioral health experience does not apply
• Strong background in care coordination, discharge planning, and utilization review
• Ability to organize and facilitate family meetings
• Knowledge of community resources for patients experiencing homelessness, mental health issues, and substance abuse
• Competent in managing durable medical equipment (DME), skilled nursing facilities (SNF), and transport arrangements
• Experience with HMOs, IPAs, and familiarity with adoption/foster care processes

Patient Care and Environment

• Patient Ratios: 25 patients per day
• Patient Age Groups: Neonates, adolescents, adults, and geriatrics
• Daily Census: Approximately 25 patients
• Interdisciplinary Support: Collaboration with physical therapy and social services

Required Skills

• Strong assessment skills to evaluate patient needs effectively
• Ability to contribute effectively to treatment planning and patient care coordination
• Proficient in computerized documentation and utilizing electronic health records (EHR) systems, preferably Epic
• Familiarity with CMS regulations and Joint Commission standards

Commitment and Expectations

• Must commit to working a minimum of 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Willingness to float between nearby facilities as needed
• Responsibility for completing required training modules prior to start and annually thereafter
• No call-ins; direct outreach to facility contacts without prior agreement is not permitted
