About the Role:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, performing vital signs, and assisting with daily living activities

• Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient

• Maintain accurate and detailed records of patient care and progress, ensuring compliance with regulatory requirements and facility policies

Requirements:

• Current LPN licensure in the state of Pennsylvania

• 0-2 years of experience in a long-term care or rehabilitation setting

• Basic Life Support (BLS) certification

• Ability to lift, transfer, and reposition patients as needed

• Excellent communication and interpersonal skills
