Actively participates in problem solving and conflict resolution to maintain team cohesiveness.Performs on-call duties effectively with the ability to triage patients over the phone and visit when needed.Directs own educational needs for the top 10 ICD9 codes and/or diagnoses for SGH HHC: CHF, COPD, diabetes, wounds/post-surgical, arthritis, newborns, Coumadin dependency, malnutrition/ nutrition needs, medication instruction, pain and symptom control/hospice care.Supervises home health aide in compliance with the Medicare/Medicaid rules and regulations.Performs other functions as required.Qualifications:· State of New Mexico or Compact State Nursing License required· Associate degree, ADN· Registered Nurse· BLS certification required at time of hireWe're all about well-being, starting with yours.
