Overview
Physical Therapist – General in Ukiah, California. Start date 09/21/2026 for a 13-week travel assignment with weekly pay ranging from $2,102 to $2,175 and guaranteed hours of 40 per week. Shift is day shift (5x8) with hours 8:00 AM–4:30 PM in a short-term acute care hospital setting through Healthforce Healthcare LLC.

Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical function, collaborating as a co-leader in an interdisciplinary patient care approach.
- Perform evaluations, establish individualized treatment plans, and deliver therapy interventions using appropriate protocols and techniques.
- Develop care and discharge plans; communicate plans to patients and other team members; ensure timely clinical documentation.
- Supervise assigned therapy assistants and contribute to continuous quality improvement within the department.
- Maintain comprehensive documentation within established deadlines and use the electronic medical record for progress notes and communication with the care team.

Requirements
- Doctor of Physical Therapy (DPT) degree; license required in the state of practice (only if license obtained on or after January 2016; foreign equivalent acceptable).
- Current physical therapy licensure in the state of practice.
- CPR/BLS certification from an approved vendor per policy (Basic Life Support).
- EPIC EMR experience is required.
- Pelvic floor therapy experience and PRPC or WCS certification (or working toward certification) are preferred.
- Minimum two years of recent physical therapy experience; 3–5 years of experience preferred; ability to work in a short-term acute care setting.
