Roles & Responsibilities

• Coordinate patient care plans across multidisciplinary teams
• Advocate for patients and families throughout their healthcare journey
• Ensure smooth transitions between care settings
• Collaborate with physicians and specialists to optimize outcomes
• Document and track patient progress with attention to detail

Knowledge & Skills

• Patient advocacy and care coordination
• Discharge planning expertise
• Electronic medical records proficiency
• Knowledge of healthcare regulations and compliance

Requirements

• Active RN license in Florida or compact state
• Case Management experience preferred
• Strong communication and organizational skills
• Ability to work independently and as part of a team
