White County Rehabilitation and Nursing is seeking a Social Services Coordinator to support residents and families and assist with care coordination, discharge planning, and admissions.

Pay: $18.00-$20.00 per hour
CNA: $19.50-$23.22 per hour

• Full-Time

• Monday-Friday

• Provide support and assistance to residents and their families

• Address resident and family concerns and help coordinate appropriate services

• Participate in care planning and interdisciplinary team meetings

• Assist with discharge planning and coordination of outside services

• Maintain accurate and timely documentation

• Assist with admissions and intake processes

• Coordinate with hospitals, referral sources, case managers, and internal departments

• Support a smooth transition for residents entering and leaving the facility

• Experience in social services, skilled nursing, long-term care, or healthcare preferred

• Experience with admissions or discharge planning preferred

• CNA certification preferred

• Bachelor's degree in Social Work or a related field preferred

• Strong communication, organizational, and interpersonal skills

• Ability to work effectively with residents, families, and an interdisciplinary team
