Nurse Practioner for Internal Medicine/ PCP

Clinical Responsibilities Perform comprehensive history and physical examinations for new and established patients. Assess, diagnose, and manage acute and chronic medical conditions including, but not limited to: Hypertension Diabetes mellitus Hyperlipidemia COPD and asthma Congestive heart failure Thyroid disorders Chronic kidney disease Obesity Arthritis and chronic pain conditions Anxiety and depression Wound care and skin conditions, if applicable Develop and implement individualized treatment plans. Order, perform, and interpret diagnostic tests including laboratory studies, EKGs, imaging, and point-of-care testing. Prescribe medications and treatments in accordance with state regulations and practice protocols. Monitor medication effectiveness, side effects, and patient compliance. Provide preventive care services including: Medicare Annual Wellness Visits Routine physical examinations Vaccinations and immunizations Smoking cessation counseling Weight management counseling Preventive screenings Conduct follow-up evaluations and transitional care management visits. Recognize conditions requiring physician involvement, specialist referral, hospitalization, or emergency intervention. Coordinate care with specialists, hospitals, home health agencies, pharmacies, and ancillary providers. Participate in chronic care management (CCM), remote patient monitoring (RPM), and transitional care management (TCM) programs as applicable. Documentation and Compliance Responsibilities Complete timely, accurate, and Medicare‑compliant documentation in the electronic health record (Eclinical experience is a plus). Ensure proper ICD‑10, CPT, and evaluation and management (E/M) coding practices. Maintain compliance with: CMS guidelines HIPAA regulations OSHA standards State Board of Nursing regulations Practice policies and procedures Review and respond to laboratory results, imaging studies, and patient messages promptly. Assist with quality improvement initiatives and clinical audits. Maintain accurate medication reconciliation and problem lists. Patient Care and Communication Educate patients and families regarding diagnoses, medications, treatment plans, disease prevention, and lifestyle modifications. Promote patient engagement and adherence to treatment plans. Demonstrate compassionate, patient‑centered communication. Address patient concerns professionally and efficiently. Maintain confidentiality and professionalism at all times. Administrative and Operational Duties Collaborate with clinical and administrative staff to optimize patient flow and scheduling. Participate in staff meetings, clinical training, and continuing education. Assist with protocol development and implementation. Support practice goals related to patient satisfaction, quality metrics, and operational efficiency. Participate in on‑call coverage as assigned, if applicable. Required Qualifications Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program. Current unrestricted Nurse Practitioner/APRN license in the State of Florida. National board certification as: Family Nurse Practitioner (FNP‑BC/FNP‑C) Current DEA registration and controlled substance prescribing authority. Current BLS certification; ACLS preferred. Minimum of 1–2 years of experience in internal medicine, primary care, family medicine, geriatrics, or related field preferred. Proficiency with electronic health record systems such as eClinicalWorks preferred. Preferred Skills and Competencies Strong clinical assessment and diagnostic skills. Knowledge of chronic disease management and preventive medicine. Excellent communication and interpersonal skills. Ability to work independently while collaborating effectively with supervising physicians and staff. Strong organizational skills and attention to detail. Ability to manage a high‑volume patient schedule efficiently. Knowledge of Medicare documentation and compliance standards. Experience with value‑based care models and quality reporting preferred. Work Environment Outpatient internal medicine and primary care clinical setting. Exposure to communicable diseases and bodily fluids consistent with healthcare environment. Fast‑paced medical office environment requiring multitasking and critical thinking. Performance Expectations Maintain high patient satisfaction scores.
