Physical Therapist | SNF Travel Assignment Fair Lawn, New Jersey | Start date 06/15/2026 | 11 weeks | 35 guaranteed hours per week | Shift: 5x8 Days (08:00–16:30) | Travel - Temporary Assignment | Skilled Nursing Facility This assignment offers an urgent‑start, increased‑rate opportunity for a Physical Therapist to provide skilled rehabilitation in a long‑term care setting. The engagement is travel‑based, with a guaranteed minimum of 35 hours weekly and a consistent daytime schedule. Key Highlights Weekly pay range: $2,452–$2,629 per week Guaranteed hours: 35 per week Contract duration: 11 weeks Shift: 5 days per week, daytime hours (08:00–16:30) Start date: 06/15/2026 Setting: Skilled Nursing Facility (travel assignment) Responsibilities Perform comprehensive PT evaluations and develop individualized treatment plans for residents Implement evidence‑based physical therapy interventions, including therapeutic exercises, gait training, balance activities, and functional mobility training Monitor patient progress, adjust treatment plans, and document outcomes in the chart in accordance with regulatory requirements Coordinate care with the interdisciplinary team, including physicians, nursing staff, and family care providers, to support discharge planning and functional goals Educate patients and families on home exercise programs and safety strategies to maximize independence Ensure safety, proper equipment use, and adherence to infection control and documentation standards Requirements Active Physical Therapist license (state‑specific as applicable) Willingness to work in a Skilled Nursing Facility setting on a travel basis Availability for a 11‑week assignment beginning 06/15/2026 Ability to work the listed shift hours (08:00–16:30) and meet weekly guaranteed hours Experience in geriatric rehabilitation or SNF settings preferred Competent documentation and communication skills to support interdisciplinary care Benefits and Support Dedicated recruiter support throughout the assignment Assistance coordinating travel logistics and onboarding Regular payroll with weekly pay cycles
