Job Description
The Staff Pad has partnered with a premier hospital in Manchester, TN, to hire experienced LPN Case Managers for their ER and Med/Surg departments.

In this role, you'll coordinate patient care from admission through discharge, ensuring quality outcomes, seamless transitions, and an exceptional patient experience.

The Case Manager collaborates closely with physicians, nurses, and care teams to ensure appropriate level of care, efficient throughput, and safe patient transitions.

What You'll Do

Responsibilities

• Assess patient needs and developindividualized care plans
• Conduct utilization reviews toensure medical necessity and appropriate level-of-care decisions
• Coordinate admissions, transfers,discharge planning, and community resource referrals
• Collaborate with physicians,nurses, and interdisciplinary teams to ensure seamless care
• Identify and resolve barriers tocare transitions and patient flow
• Educate patients and families ontreatment plans and follow-up care
• Monitor patient outcomes andidentify opportunities for improvement
• Ensure compliance with hospitalpolicies, regulatory standards, documentation and payer requirements
• Advocate for patients throughouttheir healthcare journey

Qualifications

• Graduate of an accredited PracticalNursing (LPN) program
• Current LPN license (Tennessee orcompact state)
• BLS certification required, ACLSpreferred
• Minimum 1-2 years of LPNexperience, preferably in an ER/acute care or Med-Surg setting
• Previous case management orutilization review experience preferred
• Strong clinical judgement,communication, critical-thinking, and organizational skills
• Knowledge of care coordination,utilization review, and discharge planning
• Excellent teamwork abilities andable to thrive in a fast-paced high-pressure environment
