Clinical Scribe & Documentation Specialist

Oak Street Health is seeking a Medical Scribe to support clinicians with real-time documentation during patient visits in our clinics. You will observe encounters with providers, document histories, assessments and plans, and assign CPT/ICD-10 codes. You’ll also prepare After Visit Summaries and assist with daily huddles and documentation improvement to ensure accurate records for value-based care.
