RN - Case Manager Home Health (Champaign Region)

Join our team as a Home Care Registered Nurse/Field Case Manager, providing skilled home health visits across multiple counties. Key responsibilities include: coordinating care plans, performing skilled nursing assessments, managing complex wound care, PICC lines, blood draws, TPN, antibiotics, catheters, tracheostomy care, and patient education to promote independence. Qualifications: IL RN license, BLS certification, college diploma in nursing, and at least 1 year of nursing experience. Skills and duties: case management, documentation per regulatory standards, timely communication with physicians, supervision of home health aides, and adherence to federal/state guidelines.
