What You'll Do

Provide medical care to patients in skilled nursing facilities.
Conduct patient assessments, develop treatment plans, and manage ongoing care.
Collaborate with physicians, nurses, and interdisciplinary healthcare teams.
Deliver compassionate, evidence-based care focused on improving patient outcomes.

What We're Looking For

Active Massachusetts Nurse Practitioner or Physician Assistant license and DEA registration.
Strong clinical assessment, communication, and patient management skills.
Experience in post-acute care, primary care, internal medicine, hospital medicine, or emergency medicine preferred but not required.
