Responsibilities

• Provide occupational therapy evaluation and treatment to adult and geriatric residents, focusing on functional independence and safety.
• Develop, implement, and monitor individualized OT treatment plans with measurable goals and progress documentation.
• Conduct standardized assessments, re-assess as patient status changes, and adjust goals and interventions accordingly.
• Collaborate with an interdisciplinary team (nursing, therapy staff, social work, and case management) to support discharge planning and transition of care.
• Document patient status, progress, and outcomes in the medical record; ensure compliance with regulatory and facility policies.
• Educate patients and families on home programs, adaptive equipment, energy conservation, and safety strategies to support ongoing function.

Requirements

• Active Occupational Therapy license in New Mexico (or eligibility to obtain licensure)
• Ability to obtain and maintain facility credentialing and NM caregiver account requirements as applicable
• Willingness to commit to a 13-week travel assignment with guaranteed 36 hours per week
• Strong written and verbal communication skills with proficiency in documentation
• Ability to work effectively in a fast-paced, interdisciplinary care setting and maintain standards of patient safety
