We currently have a position available for an experienced Home Health Speech Language Pathologist (SLP) to join our dynamic team! We offer competitive pay and schedule flexibility. Responsibilities: Provides speech therapy services to patients according to a written physician's plan of care, including assessment, evaluation, and participation in developing the plan of care. Evaluates home environment. Delivers a full range of Speech-Language Pathology services as ordered by a physician. Enhances or maximizes the patient's communication abilities. Assists in developing and reviewing the plan of care as necessary. Prepares and submits clinical and progress summaries based on goal attainment. Participates in discharge planning for patients. Prepares and submits clinical progress summaries as directed by organizational policy.
