Position Overview

We are seeking an experienced Acute Care Case Manager for a healthcare facility located in California. The position will require the candidate to start as soon as possible and operate on a day shift schedule, working five 8-hour days from 08:00 AM to 04:30 PM.

Qualifications

• Minimum of 2 years of experience in Acute Care Case Management or Discharge Planning.
• Hospital experience required; Behavioral health experience is not applicable.
• Valid California state nursing license is required upon start date. Pending licenses will be accepted but must be secured by the start date.
• No more than 7 days of requested time off (RTO).
• Weekend availability is necessary to meet departmental needs (every other weekend).

Responsibilities

• Conduct assessments and develop treatment plans for patients.
• Coordinate care and facilitate discharge planning processes.
• Participate in family meetings and contribute to interdisciplinary treatment teams.
• Engage with patients with diverse needs, including those experiencing homelessness, mental health challenges, and substance abuse.
• Arrange necessary medical equipment (DME), skilled nursing facility (SNF) placements, and transportation needs independently.

Additional Information

The selected candidate will work with interdisciplinary teams including Physical Therapy and Social Services. Understanding of standards set by the Centers for Medicare and Medicaid Services (CMS) and The Joint Commission is essential.

Previous experience with HMOs and IPAs is beneficial.
