Requirements

Qualifications:

EDUCATION and/or EXPERIENCE

• The Physical Therapist must have completed training in accordance with minimum essential for physical therapy schools as prescribed by Council on Medical Education and Hospitals of American Medical Association.
• He/she must graduate from an AMA-APTA approved school of physical therapy.
• The Physical Therapist must be a member or eligible for membership in the American Physical Therapy Association or American Registry of Physical Therapy.
• Must hold active license for the state of Colorado.

CERTIFICATES, LICENSES, REGISTRATIONS

1. Current, active Physical Therapist licensure for Colorado or a permit from the licensing board while awaiting licensure examination.

2. Current CPR Certification.
