Recovery Case Manager - Substance Use & Aftercare
Bay Cove Cape Cod
Bay Cove Human Services in Greater Boston and Southeast Massachusetts is seeking a Case Manager to support individuals with substance use and co-occurring disorders, coordinating services and providing counseling and aftercare planning. You will conduct psychosocial assessments, facilitate individual and group sessions, arrange referrals, and document progress in the Electronic Health Record. This is a non-exempt, on-site role with opportunities to collaborate within a multidisciplinary team and #J-18808-Ljbffr Bay Cove Cape Cod
