Crisis & Triage Licensed Clinical Social Worker

Kaiser Permanente seeks a Licensed Clinical Social Worker specializing in Crisis & Triage Management to support our Healthcare & Medical Services department. In this role, you will provide rapid psychosocial assessments, suicide and risk evaluations, and safety planning for members experiencing acute behavioral health crises. You will triage level of care, coordinate urgent referrals, and deliver brief, evidence-based interventions in close collaboration with psychiatrists, nurses, and primary care providers. The position operates in a hybrid environment with both on-site and telehealth services, requiring strong communication, documentation, and teamwork skills. You will contribute to continuous improvement initiatives that enhance access, patient outcomes, and integrated care.
