CASE MANAGER

Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.

SCHEDULE:

Monday through Friday - 9:00am-5:00pm

2 Evenings per week required to meet needs of client schedules.

SUMMARY:

Family Service League is seeking a full-time The Case Manager develops individualized planning to support children and young adults with mental illness and/or chronic medical conditions in achieving their goals. Responsibilities include conducting home visits and assessments, providing linkages, and assisting with navigating service systems.

RESPONSIBILITIES:

Outreach and engage clients with a primary history of mental illness.

Develop Strength’s Assessment with the client.

Develop crisis/safety planning in collaboration with mental health providers.

Develop individualized planning with the client in the areas of health, education, mental health, vocational, socialization, benefits, legal and life skills.

Meet a minimum of 2 times per month to support the youth in achieving their goals.

Advocate for clients and assist in the coordination and linkage of services with providers, benefit systems and the community.

Help clients navigate service systems.

Initiate crisis intervention with linkages to appropriate mental health or health services, as isnecessary.

Report all client incidents immediately.

Inform Supervisor/Director when other providers are not collaborating in the effort to achieve services for the client, or have plans to inappropriately discharge a client from housing.

Follow guidelines set by the Office of Mental Health and the Division of Mental Hygiene SPOA unit Risk Management.

Work within a team approach with Family Peer Support and Youth Peer Support workers.

QUALIFICATIONS:

A Bachelor’s degree in Social Work and 2 years’ of relevant experience, or a Master’s degree and 1 year of relevant experience is required.

Proficient computer skills, including Microsoft Office required.

Excellent interpersonal, and verbal and written communication skills, are required.

Ability to work individually and as part of a team is required.

Ability to multi-task and work in a fast-paced environment is required.

Valid and clean Driver’s License and own a reliable car to drive from worksite to worksite.

PHYSICAL REQUIREMENTS:

This position requires sitting for long periods of time and travelling within the community to conduct home visits.
