Somatus is seeking an RN Care Manager to focus on high-need CKD/ESKD patients. You will work with patients, nephrology practices, and the care team to deliver coordinated, high-quality care in home, clinic, and facility settings. You will plan and execute individualized care plans, educate patients, and collaborate with physicians to improve outcomes while supporting transitions of care and reducing readmissions.
