Registered Nurse (RN)

DWC seeks to bring on board a Registered Nurse who can provide our residents with the focused support they need to address chronic health challenges. The RN will work with residents to assess pain, mood, strength/balance, risk of falls, incontinence, or fatigue, among other chronic health symptoms. The RN may also assist residents with medication support (such as reviewing drug lists for contradictions or duplications), report changes to a resident’s health or medication to primary care provider or help them navigate a complicated healthcare system to obtain specialty care. Broadly, the RN will also coordinate a resident’s overall care, while also presenting strategies to help self-manage chronic health conditions at home including diabetes, arthritis, and hypertension. The RN will also help DWC better understand the most prevalent types of chronic health conditions among our target population, and what kinds of additional support can be integrated into our Permanent Supportive Housing program to ensure that our residents are stable and able to safely age in place by addressing the chronic health and mental health challenges that pose threats to their stability. Essential Functions Assess residents for chronic health, environment, and ability challenges that may threaten their ability to safely age in place, and connect to appropriate resources Develop trust and rapport with residents and provide services using an empathetic and client centered approach Approach residents and peers with a trauma informed lens Collaborate with residents to complete assessments; identify resources and determine appropriate treatment goals Collaborate with Permanent Supportive Housing Program Case Managers and administrative staff to support a continuum of care Collaborate with OT to ensure each resident has the ability to safely age in place, and connect to appropriate resources Collaborate with physicians to ensure continuity of care for each resident and ensure resources and services are received Provide in-unit care to older adult residents and individualized care coordination including medication support, chronic disease management, and additional support that is tailored to individual needs Provide treatment via group or individual intervention with a focus on increasing social, cognitive and functional living skills to support housing retention Adheres to all program guidelines, policies and protocols and actively works toward achieving the goals of the program Maintain accurate and timely records of activities, progress notes, and other services provided to each resident and input resident data and progress notes into various tracking systems Participate in weekly case consultation and provide feedback on the cases reviewed Coordinate health trainings and workshops to program staff Build up an infrastructure of medical assessments and/or policies or protocols aligned with Aging In Place service model with the adaptation of CAPABLE. Participate in community meetings and events and serve as an ambassador for DWC in the community Provide monthly written reports to the Permanent Supportive Program Manager Additional duties as assigned Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Work Environment Downtown Women’s Center operates several program and administrative locations in the Skid Row neighborhood of Downtown Los Angeles. The organization’s primary hub is located on San Pedro Street (SP) and includes participant-facing services, staff and departmental offices, and Permanent Supportive Housing units. Programs at the SP site include the Day Center, Women’s Health Clinic, kitchen and meal services, Trauma Recovery Center, supportive service offices, and housing units, among other operations. Although some positions may have a primary work location at another DWC program site or within the community, employees in all roles may be expected to attend trainings, meetings, or other work-related activities at the San Pedro (SP) location. Parking near the San Pedro (SP) Access Center is limited, and employees should plan for offsite parking with a short walk to the site. Physical Demands Ability to walk moderate distances between DWC sites and throughout the surrounding community. Ability to move frequently throughout program sites, including standing for prolonged periods of time. Ability to ascend and descend multiple flights of stairs and navigate multi-level buildings. Ability to frequently stand, bend, reach, and access files, equipment, and office or program supplies. Ability to sit for periods of time while completing documentation and working on a computer. Ability to lift up to 15 pounds (e.g., binders, reports, supplies, or office materials). Ability to maintain focus and attention to detail for extended periods of time. Position Type and Expected Hours of Work This is a full-time, non-exempt, on-site position. This role holds the responsibility to report on-site 5 days a week, 8:00 am to 4:30 pm, Monday through Friday. This position requires flexibility to support evenings and weekend shifts when necessary. Please note that the Permanent Supportive Housing Program operates and is staffed daily, 24hours a day and 7 days a week. Work Location This position is site-based and requires regular, in-person presence at the work location. Remote or hybrid work arrangements are not available for this role, as essential job functions must be performed on-site to effectively support staff, residents, and program operations. Travel Occasional local travel is required for meetings, program-related activities, and coordination with community partners. This role may also involve transporting clients as needed, in accordance with agency policies. Periodic travel to attend conferences, training, or professional development opportunities may be required. Required Education And Experience Valid Nursing license required Bachelor’s degree in nursing preferred. At least 1-year experience working in health care, homeless services, women and/or unsheltered, unhoused populations Willingness to work a flexible schedule and to fill in when needed Ability to explain treatments and conditions to women of all ages Working knowledge of health care laws and regulations, patient rights and responsibilities 2+ years’ experience with women experiencing homelessness and knowledge of community resources preferred Excellent bedside manner and communication skills Experience working with aging population preferred Ability to conduct psychological, psychosocial, cultural spiritual and cognitive assessments Ability to communicate effectively while maintaining good working relationships with co-workers, managers and other staff Required Knowledge/Skills/Position Qualifications Familiarity with supportive housing or other residential or shelter facilities in Los Angeles strongly preferred Knowledge of issues related to women’s homelessness, mental illness, chronic health issues, and older adults preferred Ability to exercise and model good judgment, active listening and crisis intervention skills Strong written and oral communication skills required; proven ability to communicate with diverse audiences Strong multi-tasking skills and proficiency in balancing direct service responsibilities with data management responsibilities Strong interpersonal skills and ability to collaborate with a diverse group of staff, interns and volunteers Detail-oriented, highly organized, able to manage multiple tasks and priorities, and set and meet goals and deadlines in fast paced environments Familiarity with permanent supportive housing model or other residential or shelter facilities in Los Angeles preferred Demonstrated success in working as a member of a multidisciplinary team and developing effective working relationships with staff, community-based organizations, county facilities and residents Ability to be flexible with hours when needed Knowledge of psychotropic medication HMIS experience and/or knowledge of data collection tools to track client progress preferred Cultural humility Additional Eligibility Qualifications Criminal background check Current valid California state driver’s license, insurance, with good driving record Oral and written fluency in Spanish and English a plus Cultural humility. Must be a CA resident and live no more than 80 miles from DWC; must be available for in-person meetings at DWC. Must provide documentation of a negative TB test result within the last twelve months, on first day of hire.
