RN LTSS Care Manager: Care Coordination & Outcomes

Molina Healthcare is seeking an RN Care Manager to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate integrated member care and drive toward outcomes within the continuum of services. Responsibilities include assessments, waiver processes, and care plan development, with emphasis on proactive monitoring and service integration to improve quality and reduce costs.
