Social Care Navigator (SCN)

At HDSW the Social Care Navigator (SCN) works directly with individuals to identify needs and social support that promote stability, wellness, and independence. This role bridges the gap between clients and the complex social service systems. Overview Position Overview: The Social Care Navigator (SCN) identifies needs and social supports to promote stability, wellness, and independence, bridging clients with the social service system. Responsibilities Conduct person-centered, strengths-based assessments to identify needs related to housing, food access, nutrition and transportation Facilitate referrals and warm handoffs to partnering agencies Ensure continuity of care and follow up on referrals Track client progress and document services Educate clients about available benefits and rights Complete all documentation in accordance with OMH, DOH, HUD and HDSW standards Track outcomes and service utilization for reporting Qualifications Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Public Health, or a related field with 2+ years of experience in case management, peer support, care coordination, or social service navigation OR associate degree with 4+ years of experience in case management, peer support, care coordination, or social service navigation Valid driver’s license required Bi-lingual English and Spanish a plus NYS Justice Center Criminal History Background Check clearance required NYS Office of Children and Family Services for instances of child abuse and/or neglect clearance required Must be legally eligible to work in the United States Strong communication, engagement and motivational interviewing skills Ability to work effectively with diverse populations Excellent organization, time management, and follow-through Knowledge of social service systems and community resources Familiarity with UNITEUS or similar EHR systems Salary and Benefits Salary Range: $23.07-$24.03 per hour Sign on Bonus: $3000 paid out over first year of satisfactory employment Medical, dental, and vision insurance 401k retirement benefits with employer matching at one year of employment up to 2% Paid time off including holidays, vacation, personal and sick Life & disability insurance Aflac US Alliance Credit Union Tuition Reimbursement Equal Opportunity Employer HDSW is an Equal Opportunity Employer #J-18808-Ljbffr Human Development Services of Westchester, Inc (HDSW)
