Healthcare Social Worker & Care Navigator

Optum

Optum is seeking a Social Worker in partnership with ProHealth Care to coordinate patient resources and provide direct social work services to patients and families. The role requires a BSW, 2+ years in healthcare, and on-site work in the Waukesha area. Preferred candidates hold a Master’s in Social Work and have experience in hospital discharge planning.
