Job Details Level: Entry Location: Austin Family Health Center - CHICAGO, IL 60644 Position Type: Full Time Education Level: 4 Year Degree Salary Range: $31.75 – $35.35 Hourly Travel Percentage: Local – Location to Location Job Shift: Any Job Category: Nurse PCC Community Wellness Center is seeking compassionate, mission-driven Registered Nurses to support patient care across our Chicago and near‑west suburban health centers. This role provides clinical and administrative support to providers and care teams while ensuring an exceptional patient experience. Nurses will work across various PCC locations, including Austin, ARC, Steinmetz, Salud, West Town/Boulevard, Lake Street, Melrose Park, Berwyn (PCC South), and West Suburban Hospital sites, based on staffing needs. Essential Duties & Responsibilities Report to: Director of Team Nursing Position Summary: Deliver high‑quality patient care by triaging calls and walk‑in patients, supporting smooth patient flow, and assisting providers with exams, treatments, and procedures. Perform and document patient assessments, administer medications, conduct basic lab tests, provide patient education, and ensure accurate EMR documentation. Support quality improvement efforts, maintain a safe and organized clinical environment, and collaborate with the care team. Triage emergency and non‑emergency phone calls and walk‑in patients, providing appropriate direction. Direct patient flow and assist other staff to maintain it. Assist providers with treatments and procedures as directed. Assess patients’ condition and learning needs, utilizing teach‑back methods for education. Administer prescribed medications and treatments following PCC policies and procedures. Obtain vital signs and perform routine functions to prepare patients for examinations or procedures. Maintain accurate EMR and report pertinent information to the provider. Perform laboratory and diagnostic tests as ordered by the provider. Ensure the work area is stocked with necessary supplies and materials. Perform and record waived testing competency. Assess emergency equipment, ensuring it is operational, and report any malfunction. Cooperate with other healthcare personnel to coordinate patient care and establish mutual goals. Notify patients by telephone or certified mail of abnormal test results as directed by the provider. Assist clinical staff with the orientation of new staff. Assess educational needs of clinical staff and provide additional training as needed. Initiate and pursue a plan for continued professional growth. Maintain patient confidentiality and comply with PCC policies, procedures, and safety practices. Serve as a resource for all clinical and non‑clinical support staff. Keep clinic work area neat and orderly. Follow PCC customer service guidelines and protocols. Drive continual quality improvement through participation in organizational quality improvement efforts, including but not limited to patient satisfaction, clinical, and operational initiatives. Perform other duties as assigned. Qualifications Graduate of an accredited school of nursing. 2–3 years of clinical experience preferred. 2–3 years of EMR experience preferred. Proficiency in MS Office Business Applications (Outlook, Word, PowerPoint, Excel). Current and valid State of Illinois RN license. Current and valid CPR card. Physical Demands Remain stationary 50% of the time; move around clinic site 50% of the time. Operate computer, printer, copy machine, and telephone constantly. Occasionally lift up to 10 pounds from one level to another. Transport from one site to another and cover other shifts as necessary. Other Skills Proficient reading and writing in English. Strong communication skills with patients, families, and providers. Follow‑through responsibility and good judgment. Maintain professionalism under stressful situations. Self‑motivated and directed with ability to prioritize and work efficiently under pressure. Detail oriented and able to work with minimal supervision.
