Qualicare SWSV offers home care services that focus on providing the best quality of life for our clients and peace of mind for their families. Job Summary The Care Coordinator is accountable and responsible for evaluating client needs, overseeing and monitoring the care delivered, developing an appropriate plan of care, and ensuring the competency of caregivers. The Care Coordinator commits to participate in establishing and implementing the highest standards of care. The Care Coordinator provides leadership and direction in all aspects of client care. She/he functions and participates in the development, implementation, monitoring, and evaluation of client programs and services while upholding Qualicare brand standards. This individual will assist in the delivery of home care services including client assessments, health education/counseling, and follow-up care.
