divh2Nurse Practitioner Position/h2pEssential Job Functions:/pulliManages the physical and psychological problems of the patient./liliIdentifies normal and abnormal findings./liliImplements an individualized plan of treatment, reassess and revises plan as needed./liliEvaluates the physical and emotional responses to medications and treatments./liliPrescribes immunizations per recommended schedules./liliSeeks appropriate consultation./liliCommunicates effectively with the physician and other healthcare providers to achieve desired outcomes and assure a continuous flow of patient care./liliInitiates and maintains the Problem/Medication list at each encounter./liliComplies with completeness, accuracy, and timeliness of documentation at each encounter./liliAssesses the patient and facilitates consultation when indicated./liliDirects patients to appropriate level of care and communicates all pertinent information./liliAssists in identifying and utilizing resources and/or social agencies for further treatment or rehabilitation./liliDemonstrates Leadership by utilizing the concepts inherent to the practice of the professional nurse./liliDemonstrates an ability to solve problems independently utilizing critical thinking skills and seeks assistance from immediate supervisor when appropriate./liliDelegates patient care appropriately./liliMaintains accountability for actions taken./liliEffectively functions as a resource person./liliActs as a patient advocate./liliPerforms effectively in the role of preceptor/mentor to new hires and /or students./liliAssists in maintaining / decreasing organizational/department costs./liliCompletes all competency requirements annually./liliProvides for patient safety in compliance with hospital and physician practices policies./liliAdheres to infection control processes, including standard precautions, to reduce the risk of acquiring or transmitting infections./liliUtilizes two forms of patient identification before implementing any form of patient care./liliPractices all National Patient Safety Goals./liliAssists in the departments compliance with all regulatory agencies./liliParticipates in continuous survey readiness for inspections and surveys./liliParticipates in the non-punitive medication reporting system./liliEnsures accountability for narcotics per regulatory/organizational policies./liliParticipates in office fire, safety, utility, hazard, and disaster drills as required./liliParticipates in on-call, weekend coverage, and first assist responsibilities as required./li/ulpSpecialty Job Functions:/pulliPlease see specialty specific Addendum./li/ulpRequired Qualifications:/pulliThe successful completion of an approved nurse practitioner program./liliBachelors Degree in Nursing./liliStrong leadership ability, good organizational skills, independent thinking and decision-making ability, analytical problem solving skills, sound judgment, excellent oral and written communication skills, must be able to function in a team environment, and knowledge of legal aspects and liability of nursing practice./liliDemonstrate the skills necessary to provide care and appropriately treat patients./liliProvide the care needs as described in the department policy and procedures./li/ulpPreferred Qualifications:/pulliMasters Degree in Nursing preferred./liliProfessional nursing experience and/or nurse practitioner experience preferred./li/ulpLicense, Certification Clearances:/pulliCurrent licensure to practice as a Registered Nurse in the State of Pennsylvania./liliLicensed as a Certified Registered Nurse Practitioner (CRNP) with prescriptive authority certified by a national certifying board accepted by Excela Health./liliCurrent DEA registration./liliCurrent Basic Life Support (BLS) required./liliPA Act 153 Clearance./li/ulpSupervisory Responsibilities:/pulliThis position has no direct supervisory responsibilities, but may serve as a coach and mentor for other positions in the department./li/ulpPosition Type/Expected Hours of Work:/pulliRegular, consistent, on-site and timely attendance./liliIncumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.)./liliTravel may be expected locally between Excela Health locations./li/ulpLEAN:/pulliActively promotes a Lean work culture by performing team member duties to encourage consistent use of LEAN principles and processes, including continually seeking work process improvements.
