Were seeking a caring and experienced Nurse Practitioner to provide high-quality primary care for the senior population. Join a collaborative team focused on strong patient relationships and meaningful outcomes.

Responsibilities

Provide comprehensive primary care under physician collaboration
Assess, diagnose, and manage acute and chronic conditions
Order/interpret diagnostics and create individualized care plans
Coordinate care with specialists and document accurately in the EMR

Requirements

Active NP licensure
1+ years primary care experience preferred
Strong communication, critical-thinking, and patient-relationship skills
EMR proficiency and team-oriented mindset
