Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week assignment in the Medford, OR area.

Details

• Home health setting covering Medford, OR and surrounding areas
• Patient ratio is 1:1 with up to 6 patients visited per day
• Standard productivity requirement of 30 points
• Day shift schedule, 5x8 hours (8:00 AM - 4:30 PM)
• No weekend or on-call requirements
• No floating between units required
• Uses Epic and Oasis for charting

Requirements

• Minimum 1 year of skilled home health experience required
• Current BLS certification required
• Oasis experience required
• Must have own reliable transportation for home visits
• License required if applicable
• Primary series COVID vaccination plus one booster required

Additional Information

• Responsibilities include home exercise program development and patient/family education
• Occasional travel of 50+ miles between patient visits, though scheduling is optimized to minimize drive time
• Scrubs of any color accepted
• Free parking provided along with IRS standard mileage reimbursement
• First-time travelers will not be considered for this position
• Local candidates within a 50-75 mile radius accepted at the same pay rate
• Former Providence employees may return as travelers after a 3-month waiting period
• Facility can cancel up to one shift per pay period
