Advanced Practice Clinician -Hospital Medicine- Nocturnist-Abington Campus

Job Details The advanced practice clinician (Acute Care Nurse Practitioner / Physician Assistant) works in collaboration with and as an integral member of a multidisciplinary healthcare team. This is a Nocturnist Hospitalist position at a suburban Academic Level II Trauma Center with a diverse patient population. The clinician is responsible for overnight admissions and PRN management for non‑teaching patients in a collaborative, educational setting. Responsibilities Perform complete history and physical assessments, document findings, and order appropriate diagnostic procedures. Serve as first responder for nursing and house staff during patient status changes, orders, and procedural or resuscitative interventions. Assess risks associated with acute and complex chronically ill patients, including medication side effects, immobility, nutrition, fluid/electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures. Collaborate with the interdisciplinary team to make accurate diagnoses of acute and complex chronic conditions. Manage diagnostic tests through ordering, interpretation, performance, and supervision. Formulate differential diagnoses by priority and identify complications to order appropriate interventions. Identify expected outcomes, formulate and document a plan of care, and adapt it using evidence‑based guidelines and protocols. Prescribe diagnostic strategies and therapeutic interventions, and perform advanced procedures consistent with privileges and competency validation. Maintain ongoing communication and collaboration with the interdisciplinary healthcare team. Facilitate communication with patients, families, and staff to promote continuity of care across inpatient and outpatient settings. Document key components of patient progress via daily progress notes, transfers, discharge summaries, or clinic notes, ensuring timeliness and compliance with acute care standards. Demonstrate professional practice behaviors, including preceptor/mentoring, education of graduate students, research engagement, participation in advanced practice grand rounds, and membership in professional organizations.
