Evening Shift LPN

Responsibilities:

• Provide direct patient care, including administering medications, treating wounds, and performing vital signs

• Develop and implement individualized care plans in collaboration with the healthcare team

• Monitor and document patient progress, identifying areas for improvement and communicating changes to the interdisciplinary team

• Participate in shift report, handovers, and team huddles to ensure seamless care transitions

• Maintain accurate and thorough records of patient care, including medication administration and treatment plans

• Demonstrate professionalism, empathy, and understanding in all interactions with patients, families, and colleagues
