LTSS Care Manager | Care Coordination & Waiver Services
Molina Healthcare of Illinois
Molina Healthcare is looking for a Care Manager/Coordinator to support LTSS activities and coordinate member care across the continuum. You will work with a multidisciplinary team to ensure progress toward outcomes and align with cost-effective, quality care strategies. Responsibilities include conducting assessments, developing and monitoring care plans, and coordinating waiver services and ICT meetings.
