Deliver compassionate, patient-focused care as a Home Health Registered Nurse (RN) providing in-home services. This vital contract position specializes in home health, prioritizing high-quality care, clear patient communication, and collaboration with interdisciplinary teams. Schedule Monday through Friday, 8:00 AM 5:00 PM Key Responsibilities Perform comprehensive OASIS assessments and develop individualized care plans. Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management. Evaluate patient condition, monitor for changes, and communicate timely updates with providers and team members. Educate patients and families on disease processes, medications, and healthy living strategies. Maintain thorough, accurate documentation using Home Care Home Base (HCHB). Collaborate with therapists, social workers, and home health aides for coordinated care. Attend case conferences, staff meetings, and participate in ongoing education. Uphold all regulatory and agency standards in documentation and direct care.
